Oh no....Milli condemned...or not

I took my Milli ....she's called Bea actually.....for early MOT.....not due until 14 June. I have noticed since service in March she has been surging if you know what I mean. Generally running very well tho'. She failed on emissions....and wiper blades! I was told scary stories...."It's internal (like a bleeding heart!).....possibly not CAT but I don't know. Any ideas? I don't want to condemn her because, other than this problem she just needs rusty wheel arches dealt with. She is an X reg and has done 143,000 miles.

What to do? Help!!
